Guardian’s confidential Employee Assistance Program (EAP) has trained professionals to work with you as you search for solutions to personal and workplace issues.

Benefits Include:

All benefit-eligible MWA Associates and the members of their household have access to an Employee Assistance Program (EAP). Managed by a third-party, the EAP provides a confidential setting to address personal and professional matters which may benefit from the counseling of a qualified professional. We strongly encourage utilization of this program as a resource for tools and advice for all Associates and their families. Matters that can be addressed through the EAP include the following

    • Life Changes

    • Gambling or other addictive behaviors

    • Relationships

    • Grief

    • Balancing work and home

    • Parenting

    • Mental Health

    To contact an EAP professional, Associates can call 1-800-386-7055 or visit worklife.uprisehealth.com. Individuals that do not participate in the medical insurance plan need only to enter the code: “worklife”. Consultants are available 24 hours a day, 7 days a week.

    New Medical cards can be ordered through your online portal at www.mycigna.com. You also have access to a mobile ID card through the MyCigna Smartphone App.

    I have misplaced my ID Card, how can I order a new one?

    ID Cards are not needed to access our EAP Program.

    I recently moved, how can I update my address?

    Your address can be updated directly through ADP (www.workforcenow.adp.com). After logging in you will go to “Myself” –> “My Information” -> “Profile”. From there you can update all personal information, including your address by clicking on the “View More” icon in the Personal information Section.

    After you update your address in ADP, it will update with all benefit providers.

    I am getting married, can I add my new spouse to my coverage?

    You and your household members [even a roommate] have access to face-to-face or video-based sessions with our network of licensed employee assistance counselors. Therefore if you and your future spouse live together, they already have access to our EAP.

    How does the EAP program handle billing?
    To see a counselor at no cost, you’ll need an EAP authorization code to give to your network provider. The authorization code can be obtained through worklife.uprisehealth.com, Access code worklife

    Do I need to designate a beneficiary?

    A beneficiary does not need to be designated for the EAP. Your dependent(s) will be assumed as a beneficiary for this benefit.

    Is my Domestic Partner Eligible for the EAP?

    You and your household members [even a roommate] have access to face-to-face or video-based sessions with our network of licensed employee assistance counselors. Therefore if you and your domestic partner live together, they already have access to our EAP.

    I am leaving the plan, when does my coverage end?

    Coverage will end the last calendar day of the month.

    My child is turning 26, when does their coverage end?

    Anyone who shares the subscriber’s address, including a nanny, parent, adult aged children or in-law is eligible for the EAP coverage.
