In accordance with IRS requirements, Mariner is furnishing Form 1095-C electronically.

Your Form 1095-C provides information about the health coverage offered to you during the year. While you may use this form to assist with preparing your federal income tax return, it is not required to file your taxes.

How to Access Your Form

You may access and download your Form 1095-C at any time by logging into the ADP Portal.

Prefer a Paper Copy?

Employees may request a paper copy of their Form 1095-C at no cost. To request a copy, please contact:
